COVID-19: Advice & Updates


All applicants aged 18 and above must present evidence of having received a COVID-19 dose within one year at the time of their scheduled appointment. This is in line with the new Covid-19 vaccination Technical Instructions released by the US Centres for Disease and Control (CDC).

To ensure a smooth appointment process, be prepared to present a Proof of a complete COVID-19 vaccination taken within the last year or Proof of your COVID-19 booster dose if the last one was taken over a year ago.

Requirements:

 

  • Original International Passport
  • Complete COVID-19 Vaccination Card for 18 & above (It must have been taken within a year)
  • Eyeglasses/contact lenses, immunization records, specialist report, medication (If applicable)
  • Complete COVID-19 Vaccination Card (18 & above)
  • Interview letter
  • Immunization records (if any)
  • ATM Card (for additional Payment) or you will be directed to the bank for cash payment to cover vaccinations
  • Evidence of payment for all transactions

 

Duration of The Screening: All tests are concluded in a day except those that require additional investigations

 

Medical Report: The report will be sent to the Consulate. Applicants are to collect Vaccination records and Chest X-ray CD (for applicants aged 15years and above).

 

Passport photograph: No longer required. A digital passport photo will be captured at the clinic.

 

Tests Required Include:

 

AGE RANGE COMPONENTS
Below 2 Years Detailed Medical History, Full Physical Exam
2- 14 years Detailed Medical History, Full Physical exam, IGRA Test for TB
15 years and above Detailed Medical History, Full Physical exam, Urine NAAT for Gonorrhea, Syphilis Test and Chest X-Ray

 

Time: The registration for screening starts from 8am and ends by 11am. Collection of medical report starts from 2pm and ends by 4pm (Weekdays); 12 noon to 3pm on Saturdays & Public Holidays

 

Expiration date of reports: The report has six months expiration date if all is fine. Otherwise, 3 months in special cases.

Conditions:

 

  • Payments are to be made before appointments are confirmed
  • Payment made at booking only covers the basic fee, exact immunization fees would be determined after review of Immunization records and payment made in the clinic
  • All payments made are non-refundable.
  • If you will not be able to keep your appointment, kindly notify us via email(usmedicals@qlifefamilyclinic.com) 5-working days prior to the date. You will be rescheduled based on availability of screening slots. 
  • The screening takes 4 – 5 hours
  • Results will be ready for collection after 7 working days

 

NB: Applicant(s) should come alone with exception of dependent applicants (those who are elderly, needing a translator or children).
